Information We’ll Need on Your First Visit
To get the most benefit from your first visit, please bring a list of any medications you are taking, including over-the-counter and herbal preparations. Lists of other specialists seen are also welcome. A family member or caregiver is always welcome to come with you.
It is important to arrive 20 minutes early to your first appointment for New Patient paperwork.
