Our office is open from 8:00 a.m. to 4:30 p.m. during the week. Scheduled appointments are available during the week from 8:00 a.m. through 11:45 a.m. and from 1:00 p.m through 3:45 p.m. We are also open on Saturday for walk-ins from 8:30 to 11:30 a.m. See below for more information.
Same day appointments are available for sick and routine visits. Rather than walk in, please call the office in the morning to schedule a same day visit. This will allow you to have a scheduled, same-day visit, reducing wait time. Please call early for these appointments as they tend to fill up quickly. For complex visits that require more time, we may request that you schedule your appointment on a different day in order to give you the attention that is needed.
Saturday walk-in hours are from 8:30 a.m. to 11:30 a.m. for sick and routine visits. Saturday patients are seen on a first-come, first-served basis. We ask that you arrive at least 10 to 15 minutes prior to your scheduled appointment time. Our providers do sometimes run behind due to the necessity of caring for the patients ahead of you. It is our goal to never keep our patients waiting but please know that sometimes this is unavoidable.
If you are more than 15 minutes late for your appointment, you might be asked to reschedule for another time.
At each appointment please provide our staff with any updated insurance or contact information. In addition, please always have your photo identification available.
Medication lists, including over-the-counter supplements and lists of other specialists seen are always welcome.
New Patient Appointments
We are currently accepting new patients to the practice for all of our providers. For Drs. Oldendorf and Gonzales, we are asking that you first establish with the office by seeing one of our physician assistants. For subsequent visits you may see any of our providers. We provide evidence-based care to every patient, regardless of their health insurance. If you don’t have health insurance, please visit Healthy Capital District Initiative (HCDI) at http://www.hcdiny.org or call (518) 462-1459 for assistance in attaining health insurance.
Cancellation and No-Show Policy
We require 24 hour notification for cancelled visits. For visits that are not cancelled within 24 hours, there will be a charge of $35.00 which must be paid before the next visit. In addition, no-shows (missing an appointment without notifying the office) will also result in a charge of $35.00 for the first missed appointment and $50.00 if there are additional missed appointments. These charges must be paid prior to the next visit. The Practice reserves the right to discharge patients who have multiple violations of these requirements.
The most efficient way to refill your prescription is to call your pharmacy. Your pharmacy will submit an electronic request to our office.
Prescription refills can only be made during the week. We cannot process refills on the weekend. We will make every attempt to fulfill your request as soon as possible however please allow us two business days in order to complete your request.
In some cases we will not be able to fulfill your request for a prescription without an office visit. In that case we will attempt to contact you as soon as possible.
Medical Records Request
Patients must complete and sign the Authorization for Release of Personal Health information form in order to request a release of records. This form can be found at our office or on this website under “New Patient Forms”. The fee for release of medical records is $0.25 per page, not to exceed $25.00.
